There is a great deal of diversity in the appearance of the African-American nose. All cases must take into consideration the concerns of the patient. At Bray Plastic Surgery, a complex nasal analysis is performed for each patient. Based on this analysis, recommendation will be made to improve the appearance of the nose. The goal is to maintain the nasal airway while improving the appearance of the nose and creating a nose that fits the proportions of the face. It is also important that the patient and the Dr. Bray understand each other and that the fundamental ethic qualities not be changed.
In most African-American rhinoplasty the concern is that the nose appears too wide or too large. This is usually the result of a flat nasal dorsum, wide nasal ala, or nasal base, and an under-projected and bulbous nasal tip. In this scenario, the rhinoplasty would include dorsal augmentation, or augmentation of the roof of the nose or bridge. The tip can be treated with suture techniques to narrow the tip, as well as cartilage grafts used to provide more projection and refinement to the tip. The nasal ala, or nostrils, can be narrowed with excision of the excess nasal skin. In some cases, the nasal bones require narrowing with a technique known as osteotomies, but they must match the new tip structure.
Although the above techniques are commonly used in African-American rhinoplasty surgery, they are not used in all cases because there is a blended African-American nose with various and different appearances. A thorough consultation and analysis was provided to help the patient achieve his or her goals.
